You make your full payment to your veterinary and send your receipt with a claim form to your insurance company and they will send you a check to reimburse you for the approved services.
Approved services are usually decided upon when you choose your plan. Insurance companies will reimburse you a percentage of the cost. The percentage can be somewhere around 80% but varies depending on the company and the service. The amount can be either an actual amount, meaning based on the real dollar amount you paid for that service, or reasonable cost. Reasonable cost is the amount that the insurance company has predetermined to be acceptable based on where you live. For example, if your veterinarian charges $50 for a physical exam, if your plan covers 80% of the actual cost and that has been preset to $48 in your area then you would be reimbursed $38.40. Your area is determined by your zip code, not your veterinarian's zip code.
